I understand that childwatch services are provided only while I am present in the building and taking class, and I am not to leave the premises. their childwatch workers, officers, representatives, agents, organizers, and successors from liability as a result of personal injury or property damage occurring while the above child/children is/are in their care at Burn Boot Camp HR LLC. I, the undersigned, hereby hold harmless, waive and release Burn Boot Camp HR, LLC. Continued disruptive or inappropriate behavior will not be tolerated and the parent will be contacted.We are not responsible for lost or damaged personal items.Children must wear shoes and/or socks while in childwatch.No food, gum, or candy allowed in childwatch.If the child needs to use the restroom or needs a diaper changed, we will contact you during the workout. Children should be fed and have used the bathroom or have ahd diapers changed before entering the childwatch room.We will contact the parent if their child cries consistently for 15 minutes or more.Reservations must be made through the app (if you have one child) or a web browser (more than one child).Our goal is to keep all kids healthy and safe. Please keep children home if they are experiencing any of the following symptoms: fever, chills, cough, shortness of breath, fatigue, muscle or body aches, headache, congestion, runny nose, nausea, sore through, vomiting, or diarrhea.
Childwatch is only provided while the legal guardian is on the premises.They are the parent or legal guardian of the child(ren) 12 weeks –12 years of age.By signing this agreement, the member acknowledges and agrees that:
